We operate from the assumption and expectation that you have read both the Heartwood Day School Field Guide and Application Packet before completing this form. If you have not already done so, please read through those first before continuing. Both documents can be downloaded for free on the HDS website.
If you would like to send multiple children to HDS, please complete one (1) application form for each child.
